I understand that this is a general consultation intake form designed to help my TDOV stylist determine how much time will be needed for my appointment and to help my stylist best prepare for my first visit.

I understand that 24 hours notice is required for cancellation or rescheduling of any service appointment to allow us to accommodate more applicants.

I understand that by submitting this form it does not guarantee that my goal hair can be achieved in one session or that I will receive the appointment time or stylist desired. This will be determined based on the amount of time needed, density/length/health of my hair, and consultation results.

In consideration for receiving Services (including free or discounted Services), I release and forever discharge the Salon and all individuals and organizations involved with the event, including but not limited to: owners, employees, contractors, guest stylists, barbers, cosmetologists, volunteers, interns, assistants, educators, sponsors, landlords/property managers, and their respective agents (“Released Parties”) from any and all claims for injury, loss, damage, or liability arising out of or related to the Services or my presence at/participation in the event, INCLUDING CLAIMS ALLEGING NEGLIGENCE OF ANY RELEASED PARTY, to the fullest extent permitted by law.

I understand and accept that salon services carry inherent risks, including but not limited to: nicks/cuts/abrasions, razor burn, clipper irritation, scalp sensitivity, allergic reactions to products, minor burns from tools, slip/fall risk, and dissatisfaction with results. I voluntarily assume all risks, known and unknown, foreseeable and unforeseeable, arising from my participation.

I agree to indemnify and hold harmless the Released Parties from any claim brought by or on behalf of me (or any third party) arising out of my actions, omissions, or breach of this agreement.​​​
